Insurance In-Network Medicare Part B

I am an in-network provider Federal Medicare Part B insurance. I would be happy to electronically submit insurance claims on your behalf to Medicare. You are responsible for all co-payments and co-insurance amounts at the time of service. Insurance companies do not provide payment for missed or late cancellations. You will be responsible for payment of missed or late cancellations. Please call your insurance company prior to starting services to verify eligibility and coverage.

Insurance In-Network MHN, a Health Net Company

I am an in-network provider for MHN, a behavioral health subsidy of Health Net LLC. I would be happy to electronically submit insurance claims on your behalf to MHN. You are responsible for all co-payments and co-insurance amounts at the time of service. Insurance companies do not provide payment for missed or late cancellations. You will be responsible for payment of missed or late cancellations. Please call your insurance company prior to starting services to verify eligibility and coverage.

Insurance Out-of-Network (PPO Plans)

I can provide you with a Superbill or electronically submit a Superbill to your insurance provider if you are covered by a PPO plan. I am not contracted with any PPO plans but many patients see me using their out-of-network benefits.

I am not contracted with Tricare, United Healthcare, or Blue Cross/Shield but regularly serve as an out-of-network provider for these plans. Please call your insurance provider and ask about your out-of-network benefits to get the specific details of your coverage.

Please be aware that insurance companies do not reimburse for all conditions that may be the focus of psychotherapy. It is your responsibility to verify the specifics of your coverage.

Cash, Check and Visa/Mastercard are accepted for payment.
